Owns the question of who gets paid, on what proof, and with how much interest. Plan governance comes first. On a group life certificate under an ERISA welfare plan, the plan document and the designation on file with the plan administrator control, and the state revocation-on-divorce statute is preempted before it reaches the file; on an individual contract it is live. Interest runs under the state's own accrual rule, which in Colorado and Alabama starts at the date of death, not the day the beneficiary asked.
